In order to participate: To ensure best care and support while participating in this group, we ask that every group member be enrolled and attending individual therapy. That can be at this clinic, or another clinic. Please sign an ROI for CCP and have your therapist complete a group referral form, which can be found here. After we receive your referral form, the group facilitator will reach out to answer any questions and discuss your start date. Group can be billed to your insurance, or you can self-pay.
