Please fax to 920-364-6096 any relevant medical records and an ROI* for the patient's PCP, OB/GYN, and/or other specialist(s) allowing C&CP to request records. ROI’s are not legally required per HIPAA harmonization rules, but many organizations still request them prior to sharing medical records. Obtaining the ROI helps us expedite the process.
